Wage Range: $40.67-$69.38 per hour
Bonus up to $6,000 for those new to EvergreenHealth with 3+ years of current Social Work (SW) experience or up to $3,000 for those new to EvergreenHealth with a minimum one year of current Social Work (SW) experience

Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.



Who We Are
We focus on care coordination with patients/families and work closely with members of the interdisciplinary staffto assure that each patient receives the right care in the right setting at the right time.Our care teams are composed of the following:
  • Social Workers provide crisis intervention, behavioral health and chemical dependency assessments, screen for abuse and neglect, as well as coordinate post-discharge care. We help coordinate with skilled nursing facilities, home health, hospice, arrange for equipment, and arrange for additional services in the community.
  • Utilization Management/Case Management Nurses provide clinical information to our payers and ensure that patients are meeting medical necessity for acute services. They also partner with the Social workers in coordinating post-discharge care.
  • Clinical Documentation Specialists review documentation to ensure that the patient experience is being documented accurately. They partner with the physicians to reflect the appropriate severity of illness and level of service provision.

Job Summary
The Care Management Social Worker works in partnership with the RN Case Manager and other health care professionals to ensure continuity and coordination of care for patients and families. Provides consultation to patients / families regarding social, environmental, psychological, financial and cultural factors related to illness and hospitalization supports continuity in care management and organizes all aspects of discharge planning.

Primary Duties:
  1. Assumes daily case load of select population to provide clinical social work services to patients and families.
  2. Performs other duties as assigned.


License, Certification, Education or Experience:
REQUIREDfor the position:
  • Masters degree in Social Work
  • One of the following credentials is required: Licensed Independent Clinical Social Worker (LICSW), Licensed Advanced Social Worker (LASW), OR Social Worker Associate- need to obtain full licensure in one of the above two categories within 4 years of hire.
  • 2 3 years of social work experience in a hospital setting preferred or community setting
  • Current American Heart Association Healthcare Provider BLS certification by date of hire
  • Age-specific or behavior-specific knowledge and experience in working with specific patient populations (i.e., children, elderly, those with emotional/behavioral dysfunction) relevant to area of assignment.
  • Excellent communication skills in order to effectively communicate ensure, and provide for patient needs, and to provide a legal record of this care. Accountability for professional practice, commitment to patient care, and commitment to uphold the Patient Bill of Rights.

DESIREDfor the position:
  • Two (2) years of experience working in a hospital setting
  • Case Management or other Certification
  • NICU/FMC experience
